Can women get testosterone on HRT? Yes. Testosterone is increasingly prescribed as part of HRT for menopause, particularly for low libido, fatigue, and brain fog. However, there is no testosterone product licensed for women in the UK — it is prescribed "off-label" by menopause specialists.

Why do women need testosterone?

Women produce testosterone naturally, and levels decline during menopause. Low testosterone can cause:

  • Low libido — the most common reason testosterone is prescribed
  • Fatigue and low energy — even when oestrogen and progesterone are optimised
  • Brain fog — difficulty concentrating and poor memory
  • Low mood — particularly low motivation
  • Reduced muscle mass and bone density

The British Menopause Society (BMS) supports testosterone prescribing for women with low sexual desire that does not improve with optimised oestrogen HRT. It is recommended in their 2024 guidelines.

Which testosterone products are used?

Can I get testosterone on the NHS?

It depends on your area and GP. Some NHS menopause specialists prescribe testosterone, but many GPs are unfamiliar with it or unwilling to prescribe off-label. If your NHS GP will not prescribe testosterone:

  • Ask for a referral to an NHS menopause specialist
  • Consider a private menopause consultation — most BMS-accredited specialists prescribe testosterone
  • Your private prescriber can write to your GP requesting shared care (where the GP continues the prescription)

Monitoring

Your prescriber should check your testosterone levels:

Testosterone levels should stay within the female physiological range. Side effects from excess testosterone (acne, facial hair growth, deepening voice) are rare when properly dosed and monitored.
