Refractive Lens Exchange (RLE)

The natural lens is removed and replaced with an artificial intraocular lens (IOL). Same technique as cataract surgery but performed before a cataract develops. Eliminates the need for cataract surgery later in life.

A thin biocompatible lens is implanted behind the iris, in front of the natural lens. The natural lens is preserved (unlike RLE). Reversible — the ICL can be removed if needed.

NHS route

Cataract surgery is available on the NHS when cataracts affect daily life (typical wait 12–18 weeks). NHS cataract surgery uses standard monofocal IOLs — multifocal or toric upgrades require private payment (£500–2,500 top-up per eye at some trusts).

When private lens surgery makes sense

You are over 50 and unsuitable for laser

RLE is the primary option for over-50s wanting permanent vision correction. It also eliminates the need for future cataract surgery.

You have a very high prescription

ICL corrects -3 to -20 dioptres — beyond the range of laser surgery. Ideal for younger patients with extreme short-sightedness.

You want a premium lens on the NHS pathway

If you are having NHS cataract surgery but want a multifocal or toric IOL, some trusts allow a private top-up payment (£500–2,500/eye) for the premium lens.

What to check before booking

  • Surgeon is on the GMC specialist register for ophthalmology

How much does RLE cost?
£2,500–5,000 per eye depending on the IOL type. Monofocal lenses are lower-cost; multifocal and EDOF lenses are premium. Price includes surgery and follow-ups.
RLE vs laser eye surgery — which should I choose?
Laser surgery suits under 50s with moderate prescriptions. RLE suits over 50s or those with very high prescriptions. RLE also prevents future cataracts. Your ophthalmologist will advise.
What is an implantable contact lens?
An ICL is a thin lens implanted inside the eye, behind the iris. It corrects very high prescriptions (-3 to -20). Unlike RLE, the natural lens is preserved and the procedure is reversible.
Is lens surgery painful?
No. Both RLE and ICL are performed under local anaesthetic (eye drops). You may feel slight pressure but no pain. Mild discomfort and blurred vision are normal for 1–3 days post-surgery.
